A brief listing of your past and current coaching positions (please include organization name - e.g., Penfield Rangers, Penfield Recreation, Webster Soccer Association, etc.).
What coaching license level you may have, if any.
Current (or most recent) age group coached
Current (or most recent) team record (wins/losses/ties)
Other accomplishments (include any other training experience or certifications applicable to coaching youth sports)

What age group(s) you are interested in coaching for the Penfield Rangers
What position are you interested in (head coach or assistant coach)
Are you currently coaching your son or daughter?
